Referrals

Making a referral for occupational therapy services in the Melbourne region is easy. Simply call us on 0476 047 988 and speak to one of our friendly staff who shall discuss with you our service and answer all your questions.

For referring agencies such NDIS local area coordinators and support coordinators we all have an easy to complete NDIS referral form that can be downloaded and sent to us for priority processing.

Please all ensure all fields in the referral are complete. If you require assistance please call us during business hours Monday-Friday, 9am-5pm.

Once your referral is complete we shall be in contact to make an initial appointment and discuss your individual needs. For NDIS customers this shall involve discussing your NDIS plan, and signing a service agreement in order for our service to commence.

Our service all also extends to helping those seeking support to access the NDIS for the first time, require occupational therapy functional assessments and reports for NDIS appeals/reviews, or to support funding for their next plan.
